Membership
HOW DO YOU BECOME A MEMBER OF THE INDIANA CHAPTER?
There are two classes of membership: active and affiliate. Active members shall be Fellows or Associate Fellows of the American College of Surgeons residing in the State of Indiana who apply for membership in the Indiana Chapter and agree to comply with its By-Laws. Affiliate members shall be members of the Candidate group of the American College of Surgeons residing or practicing in the State of Indiana who apply for membership in th Indiana Chapter and agree to comply with its By-Laws.
Re-application is required annually. Forms are mailed with dues statements in December. Or, printer ready application forms may be obtained HERE and sent to:
